I authorize Marketing Hub USA LLC (the "Company") to initiate electronic debit entries (ACH) to the bank account identified above, and, if necessary, credit entries and adjustments for any debit entries made in error.
I acknowledge that the origination of ACH transactions to my account must comply with U.S. law and the NACHA Operating Rules.
For recurring authorizations, debits continue on the schedule stated above until I cancel. For one-time authorizations, a single debit is initiated on or after the date stated above.
This authorization remains in effect until the Company receives written notice of cancellation from me at least ten (10) business days before the next scheduled debit, sent to admin@marketing-hub-llc.com or to 18245 NW 68 AVE APT 115, HIALEAH, FL 33015.
I am responsible for any fees charged by my financial institution, including insufficient-funds or returned-entry fees, and the Company may charge a returned-item fee permitted by law.
